[camera] Refactor image writer

Refactor of image writer to make it more modular/extensible.

This CL does the following:
* Pulls image_writer/ out of calibration/
* Adds an image format parameter to drivers/isp/modules/dma-format
* Separates out most of the RAW12-specific logic from the image_writer source. We're hoping to keep the image writer as format-agnostic as possible.

Another nice-to-have would be for the image writer to spit out images in zircon's image definition, not just as vmos.

Tested: all the previously-written tests still pass. Specifically,
running `fx run-test camera_full_test -t image_writer_test` and
`blc_test` passes.
