[debugger] Improve breakpoint formatting.

Hook up breakpoint types to the settings system. Document breakpoint
type strings in a consistent way.

Improve formatting of breakpoints. Now only non-default values are shown
in the short listing which makes typical usage cleaner and easier to
follow. Attributes are shown in a consistent key=value format using the
setting names.
