[debugger] HW breakpoint implements ProcessBreakpoint. This setups the logic about how a thread will step over a hardware breakpoint. Because HW breakpoints can be disabled per-thread, the logic is much simpler than a software breakpoint, which needs to suspend all threads. This is not yet hooked to the overall process breakpoint.
