[exceptions] channels #7: get task handles
Adds zx_exception_get_thread() and zx_exception_get_process()
syscalls to retrieve the thread and process handles from an exception.
Test: `k ut exception`, `runtests -t exception-test`,
`runtests -t policy-test`
