[bt][host][gap] Fix race in default name initialization

gap::Adapter::Initialize now notifies its callback only when the final
steps of its visible configurations (default local name and device
class) have been initialized. This guarantees that the adapter
information that a bt-host device returns after it has been published
and is accurate and fixes a race condition where a bt-host could
initially return an empty string for the name.
