[kernel][vm] Reclamation logging improvements

Fix up some memory_watchdog and evictor logs:
 - Log free memory when escalating to OOM on allocation failure.
 - Log when we avoid OOM-ing by reclaiming everything.
 - Remove unhelpful eviction target log (remnant from one-shot
   eviction).
 - Condense evictor logs to a single line instead of four.
