[roll] Roll fuchsia [libc] Rewrite thread list management in C++

This moves the all-threads list and its maintenance into C++
code. The locking is handled with a standard libsync mutex, now
wrapped as LIBC_NAMESPACE::Mutex.

The __sanitizer_memory_snapshot implementation is modernized to
take advantage of this.
