[roll] Roll fuchsia [kernel][lib/arch] Assume constant paging level dimensions for now

While in general we might end up with a smaller last level of paging
(e.g., through virtual address extension features), we have no intention
of supporting such a case at the moment, and it simplifies things to
reflect that the dimensions of tables across all levels are the same.

(We assert on this in part in the phys layers in any case.)
