[fxfs] Add object reference counts. + Added a tombstone object record that deletes all other records. + Change the ordering of records, so that attribute sizes are adjacent to attribute extents. + Made a simple change to SkipListLayer to support seeking using Bounds::Excluded, although I ended up not needing it. + Added a default implementation for OrdLowerBound.
