[feedback] lift ChannelProviderPtr

* we also need to fetch the channel in crash reporting
* this is the first FIDL client wrapper we move under //utils/fidl
* the proposed convention is the directory and the ptr suffix to
  distinguish it from the stub server that usually has the same name
